Job Summary:
Independent Living LLC is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team on a part-time basis. The LPN will play a key role in supporting the health and well-being of residents in our Chapter 6400 ODP regulated community living arrangement by completing HRST (Health Risk Screening Tool) assessments, training staff, and assisting with medication administration.
Key Responsibilities:
HRST Administration:
- Conduct regular Health Risk Screening Tool (HRST) assessments to evaluate the health needs of residents.
- Document and monitor changes in health status and communicate findings to the appropriate team members.
- Collaborate with clinical and support staff to implement appropriate care plans based on HRST results.
Staff Training:
- Provide training to Direct Support Professionals (DSPs) and other staff on health-related topics, including but not limited to medication administration, infection control, and proper documentation.
- Ensure staff are competent in handling residents' healthcare needs, promoting a safe and healthy environment.
Medication Administration Assistance:
- Assist in the administration of medications in accordance with Chapter 6400 ODP regulations and agency policies.
- Maintain accurate records of medication administration and communicate any concerns or incidents to the nursing supervisor.
- Ensure medication storage and management are in compliance with Chapter 6400 ODP standards.
Collaboration and Compliance:
- Work closely with the RN and other clinical team members to ensure that health services provided align with Chapter 6400 ODP regulations and best practices.
- Assist with audits, inspections, and compliance checks related to healthcare services within the community living arrangement.
Other Duties:
- Support the team in any additional healthcare-related tasks as needed, ensuring the health and safety of residents at all times.
- Provide input for the development and continuous improvement of healthcare policies and procedures.
Qualifications:
- Valid LPN license in the state.
- Experience working in a healthcare or residential setting, preferably within a community living arrangement or long-term care environment.
- Knowledge of Chapter 6400 ODP regulations and HRST is a plus.
- Strong communication skills and the ability to train and mentor staff.
- Ability to work collaboratively with a team to provide high-quality care.

This is a part-time position, with flexible hours depending on the needs of the community living arrangement and agency office.
